Attendance and Cancellation Policy
Attendance
Please note that I can only reserve regularly scheduled appointment times for clients who can meet weekly. If I am reserving you a regularly scheduled appointment time (e.g. Mondays at 2 pm), I request you to be able to attend at least 70% of these sessions in order for me to continue reserving this appointment time. So for example, in a 10 week period, I would expect to see you at least 7 of those weeks.
Cancellation Policy
I have a 24 hour cancellation policy. This means if you cancel your session less than 24 hours before its scheduled time, regardless of the reason, you will be charged my missed session fee of $100.
Exceptions to this policy: I allow up to two cancellations per year if you experience acute and unexpected illness and/or inclement weather conditions that are unsafe for transport to my office. Please note: I am unable to waive this policy for other reasons that might arise including unforeseen work or school demands, transportation difficulties/travel delays, childcare coverage, change of plans, frequent illnesses requiring more than two cancellations per calendar year, etc.

Insurance Policy
(Private pay clients may disregard this section)
If you are using an insurance I accept to bill your sessions (e.g. Premera, LifeWise, First Choice Health, Kaiser Permanente) you must be aware of when your insurance plan may end. For example, if your policy ends on 12/31/2020, you must communicate this with me in advance. Insurance companies can take months before alerting the therapist that the client’s policy has ended. This means that the therapist can be paid by insurance for months worth of sessions, and then the insurance company can alert the therapist that the client’s policy in fact ended months ago, and legally the client is required to cover the cost of the “overpayment”. This cost can be around $120 per session, depending on what insurance company you have. It’s a horrible situation that nobody wants to be in. Please be aware of when your policy ends. This is especially important if you are using a spouse’s or parent’s insurance, because it’s more likely that you, the client, may not be as aware of these details. By signing below you acknowledge that it’s your responsibility to know when your insurance policy changes or ends, and that you must communicate this in advance with your therapist.
